    Supporting a continuum of care with providers across Durham Region Primary Care Provider talking to a patient The Durham Ontario Health Team (OHT) is a collaborative group of organizations and patient, family and care partner advisors working in partnership to improve access and delivery of coordinated health services. The group is comprised of partners from across the care continuum, including primary care providers, hospitals, mental health and addiction services, social support services, home and community care support services and long-term care. ​ The Ministry of Health (MOH) announced the province’s first OHTs in December 2020. The Durham OHT was among this group. ​ While the core Durham OHT partnership group is comprised of 18 signatory organizations, there is broad support from more than 70 health care partners across Durham Region, representing various organizations in the health, social, education, and private sectors.     Notre histoire L'équipe Santé Ontario de Durham (ÉSO) est un groupe collaboratif d'organisations et de conseillers des patients, des familles et des partenaires de soins travaillant en partenariat pour améliorer l'accès et la prestation de services de santé coordonnés. Executive Leadership Team (ELT) The Durham Ontario Health Team Executive Leadership Table (ELT) is responsible for developing and overseeing the implementation of the Durham Ontario Health Team’s (Durham OHT) annual operational work plans. These duties include: Reviewing and approving decisions related to the allocation and sharing of costs and resources, including funding earmarked for the Durham OHT including but not limited to human resources, capital, facilities, and costs related to supporting the work of the Durham OHT; Leading formal and informal consultation activities with members of the Durham OHT Sector Network Tables, Collaboration Council and other relevant key stakeholder groups as part of the ELT’s review and approval process; Providing Executive Sponsor level support to the Durham OHT Strategic Implementation Lead; Conseil de collaboration Communications Team Working Group A coalition of partners that is committed to a common set of principles and goals to improve health and wellness across Durham. Digital Health Team Working Group Conseil consultatif des patients, des familles et des partenaires de soins The Digital Health & Data Governance Working Group is responsible for overseeing the execution of the Harmonized Information Management Plan and set priorities and direction in the OHT’s digital health initiatives, data governance and analytical capabilities. Our Projects Durham OHT Covid-19 Response The Omicron variant had a significant impact on the volumes in the emergency departments in Durham Region which brought together a working group in late December 2021. The COVID-19 Response Working Group focused on the rapid implementation of strategic initiatives to support system planning by working collaboratively to advance Emergency Department Diversion initiatives across the Lakeridge Health System. To assist with ensuring continued access to acute care hospital services during the fifth wave, a communication strategy was developed and an urgent Call to Action was initiated resulting in expanded community-based primary care clinic hours and increased access to virtual care. The Durham Ontario Health Team hosted its first Virtual Community Town Hall meeting on January 20, 2022. The event provided an opportunity for Durham Region residents to connect with primary care leaders to address questions and concerns regarding wave five of the pandemic in addition to sharing key messages related to accessing care. Over 658 people joined the event live and 3,327 attendees participated virtually. The OHT also hosted several professional development webinars that were focused on ED Diversion, our Durham Region’s Integrated COVID-19 Response and the Introduction of Antiviral Treatment Paxlovid.

  • Patient Family and Care Partners | Durham OHT

    Our Patient Family and Care Partners Engagement and partnering with patients, families and care partners are the building blocks of all Durham OHT activities in order to improve the system of health and social care and support for Durham Region residents. Our culture is driven by the lived experience and unique local needs of patients, families and care partners through collaboration and co-design across all OHT activities. Meet our Patient Family and Care Partner Advisors (PFCPAC) Michael Abra mczuk Michael's healthcare experience was formalized by a decade of assisting staff at Lakeridge Health with their IT and capital equipment needs and the accreditation review. As well, Michael made occasional presentations to the Development Committee of the Board and the Ministry of Health to help in the realization of today's Cancer Centre in Oshawa. Michael has significant family and care partner experience with persons requiring mental health supports, persons living with dementia, cancer diagnosis and treatment, and assisting frail elderly. And as a past Board member of Durham Mental Health Services he participated in helping many in the community who are/were experiencing similar challenges. As a member of the PFCPAC, Michael's priority is improved wayfinding of the healthcare system through one connected system. Michael is a professional accountant, married with two wonderful adult children, and he has a passion for travel. Manon Le monde Manon Lemonde is an associate professor at the Faculty of Health Sciences, Ontario Tech University (branding name for the University of Ontario Institute of Technology (UOIT) in Oshawa since July 2003. Prior to joining UOIT, she was an associate professor at the School of Nursing, Laurentian University in Sudbury where she was extensively involved in the delivery of the francophone baccalaureate program in nursing since 1987. She received her BScN, MScN and her PhD in biomedical sciences from the Université of Montréal. Her dissertation is titled: Quality of Life of Patients with Advanced Stages of Lung Cancer. She has many publications and presentations related to symptoms management in cancer patients, diabetes, and social support. Since her arrival at UOIT, she has been involved in projects for new grad, late career nurses, preceptor/mentor programs, health care services for French speaking seniors, cultural competence, chronic diseases management, human health resources planning in terms of recruitment, retention and work environment, program evaluation and knowledge transfer and utilization. Manon has developed a strong commitment to university community partnerships exemplified by her work with agencies such as Durham Region Health Department and Lakeridge Health. As part of her service commitment, Manon has been involved with the Francophone Table of Durham Region and was member of the Advisory Committee on Seniors Services and Long-Term Care of the City of Toronto for six years. She also held the Chair position of the Entite 4 for 3 years. These are some of her involvements in the community and Her community work targets the exploration of health promotion, prevention programs, access to health care for minority groups. Bette Hodgins Bette Hodgins has worked in the health care sector for many years, in both the acute and community sectors. Throughout this time she has enjoyed the challenges of front-line physiotherapy to executive management, healthcare consulting and most recently, start-up operations project management for Oak Ridges Hospice. Bette has also had the opportunity to serve as a Founding member of North House Shelter, and as Board Chair of the Scugog Memorial Public Library, led the development and expansion of the new library. She was also the essential caregiver for her mother until her passing in 2022. Caring for her mother gave Bette a deep appreciation for the needs of th e frail elderly, and the necessity for providing a voice for those who can no longer advocate for themselves. This became even more apparent after her mother fractured her hip in 2021, leaving her reliant on the strength, compassion and good will of others for her day-to-day quality of life. Our healthcare system is an oxymoron that has evolved from illness care, rather than health care. It is a system that is not a system; it is a series of organizational pigeonholes that inadvertently segregate the body from the mind and spirit. However the Durham OHT has a vision of creating an integrated system that overcomes historic hurdles and supports a seamless path to well-being. Along with the other members of the PFCP Advisory Council and the whole Durham OHT, Bette wants to participate in the realization of this vision where patients, families and care partners are equal voices at the planning table. Percy D'Souza Husband. Father. Son. Brother. Friend. Maverick. An ardent advocate for the marginalized and mental health-challenged among us. Ex-severe, treatment-resistant, psychotic depressive now passionately living in the moment. Certified Peer Supporter, Project Management Professional (PMP), MBA (Ivey) with over ten years of healthcare sales and marketing experience in market-leading organizations - Pfizer, Baxter, GE Healthcare, Abbott Labs. Presently leading GraceWins Peer Support, a maverick not-for-profit mental health peer support social enterprise whose purpose is to alleviate the suffering of those who struggle with mental health challenges, enable them to overcome these challenges, realize their unique potential and live their lives to the fullest. Urica Parris Dr. Urica Parris has well over 40 years of nursing experience in public health, primary, acute, and long-term care. Urica also has experience at the executive leadership level. She has worked in the United Kingdom, Ontario, and Saskatchewan. Retired in 2017 as a registered nurse, Urica is proud to share her knowledge of the healthcare system. Urica lived experience as a patient, a patient care partner, as well as her perspective as a committed patient care partner enable her to contribute to the emerging vision of our community. At Lakeridge Health, Urica participates actively as a volunteer Patient & Family Advisor, in addition to being a care partner on many Rehabilitations Care Alliance working groups. Andrea Belanger Andrea Belanger is a Durham Region resident with a passion for family advocacy and patient engagement. Having spent a lot of time in hospital as a child and most recently as an adult heart patient, Andrea has lived experience as a patient as well as advocating for her child who was born with a rare genetic diagnosis. Being rare can be isolating and Andrea’s search for support led her to build family networks within the community; and ultimately to a new career in Family Engagement. Andrea lives in Clarington with her husband and 2 children. A program manager at Grandview Children’s Centre in Oshawa, Andrea enjoys working with families to be true partners in their child’s care team. Andrea enjoys giving back to her community and supporting change at a systems level. In addition to her role on the Durham OHT PFCPAC, she is a pending Board member of the Campbell Children’s School Board of Trustees and an alternate member of the Kawartha Pine Ridge District School Board’s Special Education Advisory Committee. The Durham OHT members ensure that patients, family members and care partners are proactively and meaningfully involved in personal care and health decision conversations with Durham OHT health care professionals and social service and support providers to support shared decision making and shared care. Discussions will include options for individualized plans of care to best reflect a patient’s needs, wants, circumstances and lived experience. The Durham OHT will also actively participate in engagement and partnership activities at the provincial level, regional level and across OHTs. Read More Single Session Intake Process(Catholic Family Services of Durham) The agency’s intake process includes a screening and single session of therapy usually within a week of reaching out. Clients are screened for appropriateness for the service and are then booked for a single session of therapy (in person or virtual). Clients for whom it is clinically indicated are placed on the waiting list for ongoing psychotherapy. Any client can return to receive another booked single session every four weeks, or until their name comes up on the wait list.     Ressources communautaires The Ontario government is building a connected health care system centered around patients, families and caregivers. These changes will strengthen local services, making it easier for patients to navigate the system and transition between providers. Ontario Health Teams are being introduced to provide a new way of organizing and delivering care that is more connected to patients in their local communities. There are 54 Ontario Health Teams across the province that have been approved through a standardized intake and assessment process. The goal is to ensure that everyone in Ontario can benefit from better coordinated, more integrated care. At this time, the ministry is supporting the expansion of provider partnerships in existing Ontario Health Teams. Under Ontario Health Teams, health care providers (including hospitals, doctors, and home and community care providers) work as one coordinated team – no matter where they provide care. Providers and organizations eligible to become an Ontario Health Team include, but are not limited to, those that provide: primary care (including inter-professional primary care and physicians) acute care (such as in-patient and ambulatory medical and surgical services, including specialist services) home care community support services mental health and addictions services health promotion and disease prevention services rehabilitation and complex care palliative care (such as hospice) residential care and short-term transitional care (such as supportive housing, long-term care homes, or retirement homes) emergency health services laboratory and diagnostic services midwifery services other social and community services and other services, as needed by the population Support for Ontario Teams Supports for Ontario Health Teams are for teams at all stages of implementation across various ministry areas and supports partners with expertise in integrated care delivery and population health. The following resources have been developed to support teams in advancing the Ontario Health Teams model: ​ Ontario Health Teams: The Path Forward - New direction for Ontario Health Teams to support their progress towards maturity. This direction promotes greater standardization, informed by lessons learned through Ontario Health Team implementation to date. Ontario Health Teams: Guidance for Health Care Providers and Organizations - This document describes the components of the Ontario Health Teams model, the expectations for Ontario Health Teams at maturity, and readiness criteria to become an Ontario Health Team. Ontario Health Team: Full Application - This revised full application is to be completed by teams invited to complete a full application and can be used by any teams to prepare for future submissions. Patient, Family and Caregiver Partnership and Engagement Strategy: Guidance Document - This document provides guidance to Ontario Health Teams as they work to establish their Patient, Family and Caregiver Partnership and Engagement Strategies. The document outlines core elements including content requirements, guiding principles, relevant examples, key resources, and the process for validation and posting once complete. The ministry undertook consultation with key stakeholders to inform the development of this document, including patients, families and caregivers. Primary Care Communications Protocol: Enabling Success through Connecting Primary Care and Physician Partners - This document provides guidance to Ontario Health Teams as they work to establish their communications protocol to connect primary care and physician partners. The ministry undertook consultation with key stakeholders to inform the development of this document, including primary care associations and representatives. Harmonized Information Management Plan (HIMP) Guidance Document - This document provides guidance to Ontario Health Teams as they develop their Harmonized Information Management Plans. Collaborative Decision-Making Arrangements (CDMAs) Guidance Document - This document provides guidance to Ontario Health Teams as they establish collaborative decision-making arrangements as a team. Ontario Health Teams: Digital Health Playbook - This Playbook can be used by Ontario Health Teams to help them understand how to build their digital health plan to support the delivery of integrated care. Ontario Health Teams: Data Supports Guidance Document – This document is supplementary to the attribution data package distributed to approved Ontario Health Teams to support their ongoing implementation efforts. It is intended to provide an overview of the patient attribution methodology used to identify the networks of patients attributed to Ontario Health Teams, an outline of new data enhancements, as well as clear guidance on how to interpret the information within the attribution data package. Patient, Family and Caregiver Declaration of Values for Ontario (Updated: July 2021) - This document is to articulate patient, family and caregiver expectations of Ontario’s health care system. The Declaration is intended to serve as a compass for the individuals and organizations who are involved in health care and reflects a summary of the principles and values that patients, families and caregivers say are important to them. The Declaration is not intended to establish, alter or affect any legal rights or obligations, and must be interpreted in a manner that is consistent with applicable law.
