The core Durham OHT partnership group is comprised of 18 signatory organizations, there is broad support from more than 70 health care partners across Durham Region, representing various organizations in the health, social, education, and private sectors.

Clarington Family Health Organization (FHO)

Provides therapy rehabilitation services in the community to assist adults and children in maximizing their function abilities within their environments.  The services are delivered by occupational therapists, physiotherapists, social workers and dieticians.

Region providing specialized programs, outpatient clinical treatment and support to children and youth with special needs and their families. * Grandview offers a variety of programs and services including: On-line Parent Support Group, Autism Tele-Practice, Tele-Therapy and Tele-Practice, Cognitive Behavioural Therapy, Telephone Follow-up and Consultation, Urgent Appointments/Clinics, Autism Virtual Assessment, Dietician Assessments, and Medical Clinics

Provides nursing (registered nurse and registered practical nursing) and personal support services to individuals in their homes, schools and throughout the community 

Through OHTs, local health care and social care and support providers work as a connected team, making it easier for people to access services and transition between providers, and empowering patients, their families and care partners in their personal health care management. As the Durham OHT, our partner organization, together with patients, family members and care partners, are creating a co-designed system with a vision of becoming one connected system of health and social care and support for residents of Durham region, filling gaps in service, reducing duplication, improving patient, family and care partner experience and demonstrating a successful integrated model of health care provision.
